Excellent exposure of the cerebellopontine angle
is obtained by an approach through the mastoid
posterior to the labyrinth. Morbidity with this
approach is much less than with the standard
suboccipital approach to the cerebellopontine
angle. The retrolabyrinthine approach has been used
primarily for selective partial section of the
trigeminal nerve for tic douloureux. Cases with
this disorder, as well as other indications for
this approach, are described in detail.

nerve, fibers distal to the ganglions are
sectioned, resulting in a high incidence of
recurrence. Demonstrated is an approach that
combines the more satisfactory results of the
suboccipital approach with the lower complication
rate of the transtemporal approach. The
retrolabyrinthine technique is shown in drawings,
anatomic models, and a surgical case.
